Delilah Alonso, MD is a board certified dermatologist who provides her patients with the most current and

Dr. Alonso is a proud alumna of Carrollton School of the Sacred Heart in Miami. She then went onto graduated
from the University of Miami with a Bachelor of Science with General Honors before receiving her Medical
Degree from the University of Miami School of Medicine. Upon graduation, Dr. Alonso completed an
internship in Internal Medicine at Jackson Memorial Hospital, as well as a residency in dermatology at the
University of Miami Department of Dermatology and Cutaneous Surgery.
Dr. Alonso remains committed to the highest standards of patient care and is actively involved in several
professional organizations so that she can remain on the leading edge of advancements in her field. She is a
member of the American Academy of Dermatology, the Miami Dermatologic Society, the American Society for
Dermatologic Surgery and the American Society for Laser Medicine and Surgery. She regularly attends
conferences to stay on top of the most effective and safest dermal filler and toxin injection technique as well as
the latest cosmetic treatment developments. Over the past twelve years she has provided a range of advanced
cosmetic dermatology treatments, including facial injectables, laser skin resurfacing, medical acne treatment and
laser hair removal. In addition, she has participated extensively in research as an investigator in over 30 clinical
trials. Dr. Alonso is proud to treat patients of all age and skin types, and has experience in pediatric, general,

Hassan Alzein, MD earned his medical degree at the University of Damascus in Syria in September of 1988 and
completed his residency in 1994 at the University of Illinois at Chicago. He is board certified and has
consistently been awarded “America’s Top Pediatrician” by Consumer Research Council of America and one of
Chicago Magazine’s Top 100 Doctors for 2014 and 2018.
Dr. Alzein gained extensive experience working as a hospitalist caring for critically ill children in Pediatric
Intensive Care Units. Through his flourishing private practice, Alzein Pediatrics, he has served the community in
the southwest suburbs for over 20 years. Alzein Pediatrics currently has five providers and two offices located in
Oak Lawn and Evergreen Park. His practice is affiliated with Advocate Christ Medical Center and Little
Company of Mary Hospital where he has served as Chairman of Pediatrics for three terms.
Dr. Alzein served as a clinical instructor for Midwestern University in Downers Grove and Olympia Fields from
1994 to 1996. More recently he is an Assistant Professor in the Department of Pediatrics at Rosalind Franklin
University of Medicine and Science in North Chicago.
Dr. Alzein’s interest in the business aspect of medicine led him to be a co-founder of the rapidly growing United
Elite Hospitalists group in 2012. This group has been providing both adult and pediatric services for several
hospitals in Chicagoland and expanding its services to include Neurology and Intensive Care.
Dr. Alzein finds Telemedicine to be a promising field for the future of pediatrics. He believes that incorporating
this technology in the medical field will allow children everywhere to have increased access to advanced
healthcare.

Mohammad Al-Khudari, M.D. is a board certified ophthalmologist and a native to the southwest suburbs of
Chicago. He attended the University of Illinois in Urbana earning a Bachelor of Science degree. Dr Al-Khudari
received his medical degree from Chicago Medical School and completed his internship at MacNeal Hospital
and the University of Chicago. He performed his residency at Stroger Memorial Hospital. Following his
residency, Dr. Al-Khudari completed a fellowship in pediatric ophthalmology at Children's Memorial Hospital/
Northwestern University.
Dr. Al-Khudari has received numerous awards including Best Research Project from the Chicago
Ophthalmology Society, Outstanding Teacher of the year and Chief Resident of Stroger Memorial Hospital. He
has authored many publications and completed research on macular degeneration, glaucoma and pediatric
ophthalmology.

Fatma Alshehri, MD is a CEO of Health and Beauty Rejuvenation clinic and spa in the heart of Riyadh, the
capital city of Saudi Arabia. She is a graduate of King Saud University and is a board certified dermatologist by
both Saudi and Arab board councils. Dr Fatma is also a holder of a King Saud fellowship in dermatologic and
laser surgery and she completed a clinical attachment for one academic year in Rothschild Hospital in Paris
where she found her passion about cosmetic dermatology, face and body contouring.
Dr Fatma worked at the military hospital as a consultant dermatologist but shifted to private practice as a part
time job in one of the elite dermatology clinics in Riyadh. Five years later she established her own clinic with a
special focus on cosmetic dermatology and anti aging medicine.
Dr Fatma is a member of many local and international societies. She is an invited speaker to conferences both
locally and internationally and she has won many prizes and awards in both social and academic levels. Dr

Michael Aronsky, M.D. is the managing partner of Kremer Eye Center in King of Prussia, PA. After graduating
from the University of Pennsylvania, Dr. Aronsky obtained his medical degree from the University of South
Florida College of Medicine. He then performed his internship in transitional medicine at St. Vincent’s Hospital
in New York. His residency in ophthalmology was performed at the Friedenwald Eye Institute in Maryland,
where he was appointed Chief Resident of Ophthalmology in 1997. Dr. Aronsky was awarded a one-year
surgery fellowship to study corneal and refractive eye surgery at the renowned Case Western Reserve University
and the University Hospitals of Cleveland.
Dr. Aronsky has performed tens of thousands of refractive surgeries, including LASIK, PRK, LASEK, Intacs

Alicia Barba, MD is a Miami-based board certified dermatologist who is a recognized authority on cosmetic

David H. Bellamah, MD, FACS, received his Bachelor of Science degree from Cincinnati, Ohio’s Xavier
University Summa Cum Laude in 1991. From there, he attended the College of Medicine at the University of
Cincinnati, which is where he received his Doctor of Medicine degree in 1996. After medical school, Dr.
Bellamah finished his surgical residency at Cincinnati’s Good Samaritan Hospital in 2000.
He relocated to Western Montana in 2001. Since that time, he has been providing the residents of Missoula,
Montana, and its surrounding communities with state-of-the-art care in General Surgery, Advanced
Laparoscopic Surgery, Wound Care, Treatment of Venous Disease, Trauma and Acute Care Surgery.
Dr. Bellamah has 15 years of experience in the field of Vascular Surgery. His passion for treating venous

Don Benefield, MD is an ophthalmologist who practices in Gulfport, Mississippi. Dr. Benefield specializes in

Don Benefield is a native of Gulfport, Mississippi and received his BS degree from the University of Southern
Mississippi in 1982. He attended Tulane Medical School then interned at Alton Ochsner Medical Foundation in
Internal Medicine. Dr. Benefield completed his training at Tulane Department of Ophthalmology in 1990.
Dr. Don Benefield founded Benefield Eye Care, PC on the beach in Gulfport in 1990. After the clinic was

Inc. six miles inland. The eight lane clinic, built in the A. Hays Town architectural style also has a LASIK suite
and eyewear shop. Dr. Benefield employs an optometrist and 23 other full and part time staff.
Dr. Don Benefield served as president of the University of Southern Mississippi International Alumni
Association in 2000. He was elected to the University of Southern Mississippi Hall of Fame in 2002.
Dr. Don Benefield is board certified by the American Board of Ophthalmology, past president of the Mississippi
Academy of Eye Physicians and Surgeons, and president of Tulane Eye Alumni. He was outstanding Tulane Eye
alumnus of the year in 2006, has received the Physicians Recognition Award from the American Medical
Association, and Lifelong Education Award from the American Academy of Ophthalmology. He is also a
clinical instructor of ophthalmology at Tulane Medical School, a member of the Refractive Surgical Alliance,
International Society of Refractive Surgery, American Society of Cataract and Refractive Surgery, New Orleans
Academy of Ophthalmology, Mississippi State Medical Association, Coast Counties Medical Society and Eye Pac

Dr. Benefield lives on the bayou in Gulfport with his wife Patricia. He has 3 grown daughters (although the youngest is
a sophomore at the University of Mississippi) and an 11 month old grandson. He enjoys boating and big game fishing.
He also likes to create, invent, build and fix things. Dr. Benefield has a patent on an eye dominance detection device
and methods. He is also an artist who occasionally creates t-shirt art for events. Dr. Benefield is looking forward to the

Craig S. Bindi, MD is owner, medical director, and the surgical ophthalmologist at the Laser Eye Center of
Silicon Valley in San Jose, California and the Laser Eye Center of the East Bay in Pleasanton, California.
Dr Bindi graduated and trained at the University of California at Davis School of Medicine, Department
of Ophthalmology Residency Program and completed an additional one-year academic post-doctoral fellowship
at the University of Arizona School of Medicine, studying and researching corneal physiology and refractive
surgery. Dr Bindi completed a general surgery & medicine internship in Tucson, Arizona.
Dr Bind has performed over 40,000 refractive surgical procedures and is an expert in the field of vision
correction surgery. Dr Bindi is the Oakland Raider’s team ophthalmologist, LASIK surgeon and sideline
physician.

Marc Bosem, MD is a comprehensive refractive surgeon practicing in South Florida and medical director and

affiliated with and a board member of the Eye Physicians of Florida a large single specialty practice with 23
locations and 40 physicians.
Marc is married to Karen Borushok, MD, a diagnostic radiologist and has two children Melissa and Brian.
Melissa is studying at the University of Chicago and Brian will be attending Duke University in the fall.
Marc is a political science and biology graduate of Emory University. He then went to the University of Miami
Medical School followed by an Ophthalmology residency in San Antonio at the University of Texas Health
Science Center.
Marc is a native of Florida with an interest in all water sports, golf hiking and snow skiing. He has traveled
extensively to Europe and South America.

Jeremy Boucher, MD, is owner of Jeremy M. Boucher, M.D., PLLC and is an attending anesthesiologist in
Oklahoma City, OK. Currently he works predominantly at the Dean McGee Eye Institute Surgery Center.
Dr Boucher is a graduate of the University of Oklahoma Health Science Center, where he graduated with
distinction, was selected for AOA, and he received the Charles Carmack Award for most outstanding student in
anesthesiology. He was also a Foundation for Anesthesia Education and Research Scholar award recipient and
the Society for Education in Anesthesiology Scholar award recipient. Subsequently, Dr Boucher completed his
Anesthesiology Residency at the same institution and was also Chief Resident. Following residency completion,
Dr Boucher did a Liver Transplantation Anesthesiology Fellowship at the Nazhi Zuhdi Transplant Institute in
Oklahoma City. He practiced Liver Transplant Anesthesiology for 7 years before assuming his current position.
As a result of his past training and current position Dr Boucher has professional interests in solid organ
transplantation anesthesiology, as well as, anesthesiology for complex ophthalmic surgery. He has interests in
medical student, resident, and fellow education, and was selected by the Dean McGee Eye Institute faculty as the
only anesthesiologist to be a member of the lecturing cadre' of physicians for the residents and fellows. He has
further professional interests in Critical Care Medicine, Sleep Medicine, and Palliative Care.
Personally Dr. Boucher has numerous avocational interests. He is a certified SCUBA Rescue Diver currently
working on becoming a Divemaster and he possesses his Bareboat Sailing License and enjoys sailing in new
destinations on vacation trips. He has multiple interests and extensive training in numerous forms of martial arts
and continues to further his instructor certifications in these multiple specialty arenas.

Francesco Carones, MD was born in Italy in 1963. He graduated in 1988 at the University of Milan, Italy, where
he attended the Department of Ophthalmology as resident. In 1992 he took a clinical training period focused
on refractive surgery at Emory University, Atlanta GA, USA. Formerly he was Assistant Professor of
Ophthalmology and Director of Refractive Surgery at the Department of Ophthalmology and Visual Sciences,
S. Raffaele Hospital, University of Milan, Italy. Now in private practice, Dr. Carones is co-founder and Medical
Director of the Carones Ophthalmology Center in Milan, Italy where he performs cataract and refractive
surgery.
Dr. Carones is involved in refractive surgery since 1989, when he started pioneering the use of excimer laser.
He participated at several clinical trials, including the first human study on hyperopia correction using erodible
masks. He explored the application of mitomycin C after laser surgery. He was among the first to have access
to the tracker-assisted flying-spot ablation technology and to propose LASEK. As for cataract surgery, Dr.
Carones pioneered the implant of multifocal and extended depth of focus IOLs, and he was among the firsts to
implement his practice with a femtosecond laser.
Currently Dr Carones is a clinical investigator for several major surgical companies. His cataract and refractive
surgery practice also involves phakic IOLs, intracorneal ring segments, corneal inlays, lens surgery, and refractive
keratotomy. Dr. Carones has published many papers in peer-reviewed scientific journals, books and book
chapters on refractive surgery and corneal topography. His research activities lead him to lecture and present at
major meetings on refractive surgery worldwide, including AAO, ASCRS, ESCRS and ISRS. He is member of
the most important Editorial Board of Journals of Ophthalmology and he currently serves as Board Member of
several national and international Ophthalmology Societies.
In his private life, Dr. Carones is interested in travelling, especially in the whole Africa continent. His principal
hobby is photography. He likes winter skiing, scuba diving and socializing.

Mohsin M. Cheema, MD is a board-certified ophthalmologist who has been serving the mid-Hudson Valley
since 1998. A former valedictorian of Kingston High School, Dr. Cheema graduated Summa Cum Laude from
Rensselaer Polytechnic Institute with a bachelor’s degree in biology. He obtained his medical degree with honors
from Albany Medical College where he was elected into the Alpha Omega Alpha Honor Society, a national

Catherine Choi, MD is an Interventional Pain Management Physical Medicine and Rehabilitation physician. She studied
at the University of Illinois College of Medicine in Chicago, IL and went on to West Suburban Hospital in Oak Park, IL
for her internship in Internal Medicine completing her physical medicine and rehabilitation training at Schwab
Rehabilitation Hospital in Chicago, IL. Enjoying working with her hands and procedural techniques as well as facing the
challenges of chronic pain management and chronic illness, she furthered her training at the University of Chicago
Department of Anesthesiology and Critical Care completing a fellowship in Interventional Pain Management. Since
completion of her training, she has worked both briefly in private practice settings and recently completed four and half
years working for a larger semi academic institution tasked with creating a chronic pain program. While she learned much
medicine through all of her training and employment opportunities, she intentionally had shied away from any business
matters with the determination of leaving those decisions to the "business folks." Growing as a physician and leader
however, she learned that these two subjects "business and medicine" are inherently married and in order to practice
medicine to the fullest potential, she would have to study business as well. This fact particularly rang true when she
attempted to grow and expand this chronic pain program in a business driven environment. She recognized the growing
trend of physician/practitioner burn out in various specialties and hopes the PCEO education may provide further
insight as to tangible methods to counter this in the business oriented environment medicine has become. Humbly, she
presents for this course to immerse in furthering education and physician empowerment with like minded colleagues.
Catherine has lived in Chicago for most of her life and is happily married with two young and energetic boys. She was the
middle of two sisters, the younger of which had a chronic illness and recently passed away in late 2016. Exposure to
chronic illness in a close family member has gifted Catherine with the additional insight into the shortcomings of
medicine and business and her personal mission is to strive to continue to educate herself further to become a better
physician. Her passion is teaching and she hopes that her life work will one day help to make a deep impact to both
patients and providers. She has made the difficult decision to leave her hometown Chicago to accept a new position in
the Twin Cities, Minnesota. Here, working again in a private practice setting, she hopes to be able to utilize her education
through the PCEO program to strengthen her role as a physician and leader.
Her personal interests include all things crafty (she likes to work with her hands and create) such as cooking, baking,
knitting, sewing in addition to running, yoga and event planning. A successful weekend always ends with a bustling home
of close friends and family enjoying fellowship and good food.

Erik Chotiner, MD, FACS, is a Phi Beta Kappa graduate of Franklin and Marshall College. He received his
medical degree from Thomas Jefferson University in Philadelphia where he was the recipient of multiple
academic, clinical, and research awards. He completed his ophthalmology residency at Temple University in
Philadelphia followed by a year long corneal/refractive fellowship at the University of California in San Diego.
In 2006, Dr. Chotiner began his private practice career at Memorial Eye Institute in Harrisburg, PA. His father,
Dr. Bennett Chotiner, had founded the Institute 30 years earlier. Memorial Eye Institute is a full service
ophthalmic practice and surgery center with a comprehensive staff of ophthalmologists, optometrists, nurses,
ophthalmic technicians and opticians. Established in 1985, it was Pennsylvania’s first fully approved, licensed,
and accredited outpatient ophthalmic surgery center.
Dr. Chotiner is certified by the American Board of Ophthalmology and is a fellow of the American College of
Surgeons. He is a Clinical Assistant Professor of Ophthalmology at The Pennsylvania State University College
of Medicine in Hershey, PA. In this role, he participates in the education and training of residents and medical
students. He is a member of numerous medical and ophthalmic societies and organizations, and is active in both
the medical and ophthalmic communities.
Dr. Chotiner specializes in anterior segment surgery with an emphasis on corneal and lens based refractive
procedures. He was one of the first physicians in Pennsylvania to perform femtosecond laser assisted cataract
surgery.
Dr. Chotiner’s avocational interests include ophthalmic videography and the clinical applications of smartphone
technology. Memorial Eye Institute’s fully equipped digital video production studio has recently been renovated
and upgraded, and supports the Institute’s clinical and educational activities.
Dr. Chotiner is an avid sports enthusiast (tennis, golf, skiing). He lives in Harrisburg with his wife Robyn and
his children Lauryn (age 11) and Michael (age 9). He enjoys attending Broadway musicals, classic rock concerts,

Y. Ralph Chu, MD is Founder and medical director of Chu Vision Institute and Chu Surgery Center. Dr. Chu is
recognized as an international leader in cataract and refractive surgery. He is a fellowship-trained corneal
specialist whose practice focuses on cataract, refractive and corneal surgery.
Dr. Chu received his medical degree from Northwestern University Medical School in Chicago, Illinois, where

Arthur Cummings, MD is a cataract and refractive surgeon and owner and medical director of the Wellington
Eye Clinic in Dublin, Ireland. He trained as a comprehensive ophthalmologist in Pretoria, South Africa and then
completed a medical retinal fellowship in Ghent, Belgium. He then went on to obtain the FRCSEd degree from
the University of Edinburgh. He relocated with his family from South Africa in 1998 to Dublin where he had
commenced a refractive surgery fellowship at the Wellington Eye Clinic. At the end of the 18 month fellowship,
when the opportunity arose, the family decided to stay on in Dublin. At this point he focused on refractive
surgery and ceased to do retinal surgery.
WaveLight Laser GmbH launched the first high speed small flying spot laser in 1999 and Dr Cummings had the
3rd WaveLight laser ever produced installed in the Wellington Eye Clinic. Thus started a long and very
productive relationship with WaveLight specifically and industry in general. Besides consulting for WaveLight
today, he also consults for Alcon in the IOL space and for TearLab.
Working with industry stimulated his appetite for research and he and a colleague started a start-up company
around a patent of theirs that allowed IOL power calculations without the use of formulae. The clinical trial
concluded successfully and the technology was acquired by a corporate.
Dr Cummings has presented more than 300 presentations at international conferences, more than 80 courses,
has written 8 chapters for various textbooks and co-edited a book on corneal cross-linking that was recently
published. He has published more than 100 articles including peer-review and trade journal articles. He is
currently serving as the European President of the American-European Congress of Ophthalmic Surgery
(AECOS) and serves on the Practice Development Committee of the ESCRS (European Society of Cataract and
Refractive Surgery). In addition to being the medical director of the Wellington Eye Clinic he is the chairman of
the department of ophthalmology at the Beacon Hospital in Dublin and the vice-chairman of the research ethics
committee. He is widely considered to be someone that is passionate about the field of ophthalmology and
specifically cataract and refractive surgery and as a result has been serving as associate chief medical editor of
Cataract and Refractive Surgery Today Europe, a widely distributed and very popular trade journal.
He has been married to Sandra for 31 years and has 2 sons, Brendan (27) and Keagan (23).

Sandra Cummings, AuD practices as an Audiological Scientist/diagnostic audiologist at Beacon Audiology. She
started the practice in 2001 and is currently the only clinic in Ireland providing fully comprehensive Audiological
and vestibular assessments for children and adults. Beacon Audiology has four full-time clinical audiologists
performing adult and paediatric hearing and balance assessments. The clinic has a 95% referral base from Ear,
Nose & Throat consultants, General practitioners, Neurologists, paediatricians and other rated medical
professionals.
Dr Cummings is currently the President of the Irish Society of Hearing Aid Audiologists. Prior to relocating to
