Welcome to PHYSICIAN CEO 2018
The physicians listed in this directory comprise the PHYSICIAN CEO class of 2018. They are an elite
group of physicians who have accepted the mantle of business leadership in addition to their professional
role as physicians.
The PHYSICIAN CEO program offers a unique opportunity to gain advanced business skills. It is offered
exclusively to physicians, and was developed by SurgiVision Consultants, Inc. in collaboration with the
Kellogg School of Management at Northwestern University.
We are fortunate to have a partner in Kellogg, which is consistently ranked among the top programs for
Executive Education in the world. Most of all, we are fortunate to work with physicians of such high
caliber as those in the 2018 PHYSICIAN CEO program. This is an outstanding group of accomplished
physicians.
Globally acclaimed faculty, a focus on CEO leadership, and a physician-oriented curriculum combine to
make the PHYSICIAN CEO program unique. As we embark on the first module, we begin of a journey

Norms and Expectations
Participants are expected to conduct themselves within acceptable norms of behavior and conduct at all times throughout the duration of the program, both on and off the campus of the James L. Allen Center and the Kellogg School of Management at Northwestern University.

Dress code is business casual.

• The PHYSICIAN CEO program is designed for professional development and interaction. Disruptive, abusive, or disrespectful behavior is prohibited and may lead to expulsion from the program at the sole discretion of the program administrators.

PUNCTUALITY, PARTICIPATION AND ATTENDANCE
• All participants are expected to actively participate in both classroom and group activities.
• Due to the carefully planned nature of the program, all participants are expected to arrive at the scheduled classes and activities on time. Late arrival may prevent entry until the next session.
CELL PHONE AND ELECTRONIC DEVICE USE
• In-class use of cell phones and computers for non-classroom related activities is prohibited. Participants who must take emergency calls are asked to leave the classroom. Re-entry into the classroom may be denied and is left to the discretion of the faculty. • No audio or videotaping of any kind is permitted of any part of the PHYSICIAN CEO program. Program Chairman
Guy M Kezirian, MD, MBA, FACS is the president of SurgiVision® Consultants, Inc. of Scottsdale, Arizona. He is a board-
certified ophthalmologist and a Fellow of the American College of Surgeons. He earned his MBA from Northwestern
University’s Kellogg School of Management, and also holds an American Society of Ophthalmic and Plastic Surgery (ASOPRS)
specialty board certification. Dr. Kezirian obtained his BSc from McGill University and his MD from the Brown University
Program in Medicine.
SurgiVision® Consultants, Inc. was founded in 1995 and is located in Scottsdale, Arizona with a mission to contribute to the
advancement of refractive surgery and ophthalmology through innovation. Over the years, SurgiVision® has fulfilled that
purpose in many dimensions, including:
· Design and administration of many successful FDA projects
· Work with manufacturers to develop new product designs
· Provision of custom, cloud-based OEM software products to monitor the safe release of new products
· Development of the SurgiVision® DataLink family of software products
· Development and administration of some of the largest privately held clinical registries for ophthalmic data
· Development and program direction for professional meetings
· Executive consulting to C-level executives

Prior to founding SurgiVision® Consultants, Inc., Dr. Kezirian started the first practice in the United States devoted exclusively
to refractive surgery in 1991, and has worked extensively throughout the ophthalmic industry. He has authored more than 20
articles for the peer-review literature and serves on the editorial advisory board of several ophthalmic trade journals.
SurgiVision®’s extensive network of affiliate consultants includes regulatory experts, software engineers, IT experts,
academics, and attorneys.
In addition to his work with SurgiVision®, PHYSICIAN CEO™ and the Refractive Surgery Alliance, Dr. Kezirian serves as a
board member of the Himalayan Cataract Project, a 501(c)(3) organization with the mission to eliminate needless blindness on
a global scale.

Academic Director
For over 24 years, Thomas M. Turmell has provided capital and advisory services to small, medium, and growth
businesses. His background includes diverse experience managing financial and operating aspects of companies,
including: investment/capital deployment analysis and execution; financial planning; strategic planning;
corporate structuring; investor negotiations/relations; lender relationship management; vendor negotiations; and
leading continuous improvement initiatives. Mr. Turmell Managing Director at TMT Capital Partners, LLC and
Director of Corporate Partnerships for Executive Education at Kellogg School of Management at
Northwestern University.
Prior to founding TMT Capital, Mr. Turmell held the position of Principal at Golub Capital, providing senior
debt, junior debt, and equity capital to middle market companies. Mr. Turmell joined the firm in 2004 to
establish and build the firm’s Chicago office, make new investments, and participate in portfolio management.
Before Golub Capital, Mr. Turmell was a Vice President with Pfingsten Partners, LLC., an operationally–
oriented, middle market private equity firm focused on making control investments in middle market companies
in the manufacturing, distribution, and publishing sectors. Before Pfingsten, Mr. Turmell was a Vice President at
LaSalle Bank, NA, providing credit and non–credit financial solutions to middle market manufacturing,
distribution and service businesses.
In addition to his activities at TMT Capital, Mr. Turmell is an Executive Committee Member of the Chicago
Chapter of the Association for Corporate Growth, and leads several committees that execute various
conferences and programs produced by the Chapter. He also serves as an Advisory Board member of Windsohy,
LLC, and Fundology, Inc.
A Michigan native, Mr. Turmell holds a BBA degree in Business Administration from the University of Notre
Dame, and an MBA degree in Finance, Management and Strategy, and Organizational Behavior from the J. L.
Kellogg Graduate School of Management.

Mr. Robert E. Grant, 48, is the Founder, Chairman and CEO of Concierge Key Health, the first on-demand app
to the world's elite physician specialists. He is also the Founder and Vice Chairman of ALPHAEON
Corporation (a subsidiary of Strathspey Crown Holdings LLC), a leader in the rapidly growing field of Lifestyle
Healthcare and based in Irvine, CA.
He is concurrently the Chairman and Managing Partner of Strathspey Crown LLC, a growth equity firm based in
Newport Beach, CA.
He was formerly CEO and President of Bausch and Lomb Surgical in June of 2010. Prior to this, he led Allergan
Medical as President from 2006 to 2010.
Mr. Grant currently serves on several corporate boards. He is an independent Board Director at Myoscience,
Inc., Acufocus, Inc. and ReShape Medical, Inc. All are leading lifestyle healthcare companies. Additionally, he
serves as Chairman of Crown Sterling LLC, an investment banking subsidiary of Strathspey Crown.
Additionally, he serves or has served on the boards of the following university and charitable organizations: The
Schmid College of Chapman University, the University of California, Irvine (UCI) CEO Roundtable Steering
Committee, The School of Law at UCI, The Henry Samueli School of Engineering, OCTANe (501-C3 non-
profit entity) an economic development corporation for technology companies in Orange County, California
where he served as Chairman of the Board of Directors, and the Young President’s Organization (YPO/WPO)

Hassan Alzein, MD earned his medical degree at the University of Damascus in Syria in September of 1988 and
completed his residency in 1994 at the University of Illinois at Chicago. He is board certified and has
consistently been awarded “America’s Top Pediatrician” by Consumer Research Council of America and one of
Chicago Magazine’s Top 100 Doctors for 2014 and 2018.
Dr. Alzein gained extensive experience working as a hospitalist caring for critically ill children in Pediatric
Intensive Care Units. Through his flourishing private practice, Alzein Pediatrics, he has served the community in
the southwest suburbs for over 20 years. Alzein Pediatrics currently has five providers and two offices located in
Oak Lawn and Evergreen Park. His practice is affiliated with Advocate Christ Medical Center and Little
Company of Mary Hospital where he has served as Chairman of Pediatrics for three terms.
Dr. Alzein served as a clinical instructor for Midwestern University in Downers Grove and Olympia Fields from
1994 to 1996. More recently he is an Assistant Professor in the Department of Pediatrics at Rosalind Franklin
University of Medicine and Science in North Chicago.
Dr. Alzein’s interest in the business aspect of medicine led him to be a co-founder of the rapidly growing United
Elite Hospitalists group in 2012. This group has been providing both adult and pediatric services for several
hospitals in Chicagoland and expanding its services to include Neurology and Intensive Care.
Dr. Alzein finds Telemedicine to be a promising field for the future of pediatrics. He believes that incorporating
this technology in the medical field will allow children everywhere to have increased access to advanced
healthcare.

Craig S. Bindi, MD is owner, medical director, and the surgical ophthalmologist at the Laser Eye Center of
Silicon Valley in San Jose, California and the Laser Eye Center of the East Bay in Pleasanton, California.
Dr Bindi graduated and trained at the University of California at Davis School of Medicine, Department
of Ophthalmology Residency Program and completed an additional one-year academic post-doctoral fellowship
at the University of Arizona School of Medicine, studying and researching corneal physiology and refractive
surgery. Dr Bindi completed a general surgery & medicine internship in Tucson, Arizona.

Gregory A. Buford, MD is Board Certified in Plastic Surgery and is a nationally recognized expert on minimally
invasive facial rejuvenation and author of “Beauty and the Business”. He has personally performed over 6000

Mohsin M. Cheema, MD is a board-certified ophthalmologist who has been serving the mid-Hudson Valley
since 1998. A former valedictorian of Kingston High School, Dr. Cheema graduated Summa Cum Laude from
Rensselaer Polytechnic Institute with a bachelor’s degree in biology. He obtained his medical degree with honors
from Albany Medical College where he was elected into the Alpha Omega Alpha Honor Society, a national
medical honor society. After completing an internship in Internal Medicine at the Albany Medical Center, Dr.

Y. Ralph Chu, MD is Founder and medical director of Chu Vision Institute and Chu Surgery Center. Dr. Chu is recog-
nized as an international leader in cataract and refractive surgery. He is a fellowship-trained corneal specialist whose prac-
tice focuses on cataract, refractive and corneal surgery.
Dr. Chu received his medical degree from Northwestern University Medical School in Chicago, Illinois, where he was a
member of the Alpha Omega Alpha Honor Society. He completed an internship at Evanston Hospital and an ophthal-
mology residency at Duke University Eye Center. Dr. Chu then completed a one-year fellowship in cornea and external
disease, refractive surgery, cataract and lens implant surgery and glaucoma in Minneapolis.

Manuel F. Del Toro, MD is a board certified ophthalmologist specializing in Cornea, External Disease and
Refractive Surgery. He is the founder, medical director, and chief refractive surgeon at VEO Clínica de
Corrección Visual in Bayamón, Puerto Rico.
Dr Del Toro received his medical degree form Ponce School of Medicine, and went on to residency at the

Gary J. L. Foster, MD graduated from Brigham Young University, Cum Laude, and then received his M.D. degree from
the University of Utah with AOA Honors. He completed his residency in Ophthalmology at the Moran Eye Center, and
scored in the top one percent nationally of board certifying ophthalmologists. He then received post-residency
training in eye laser and refractive surgery before joining the Eye Center of Northern Colorado, in Ft Collins in 1995.
Dr. Foster’s practice focuses on laser cataract correction and refractive surgery to restore vision and decrease the
need for distance and reading glasses. This is accomplished through laser cataract removal with near focusing lens
implants (ReSTOR, Technis multifocal, and Crystalens), laser eye surgery (LASIK and PRK), and phakic IOL’s.
Dr. Foster has been a principal investigator in numerous research projects to enhance eye treatment and technology
and is a certified physician trainer for the Visx laser vision correction system. He is involved in teaching advanced
surgical techniques at the American Academy of Ophthalmology (AAO) and The American Society of Cataract and
Refractive Surgery (ASCRS) national meetings, and speaks regularly on related topics to train other doctors around the
world. He is currently the on the cataract committee and co-chairs the Clinical classroom committee for the ASCRS.
The clinical classroom committee helps train third world eye surgeons in modern surgical techniques to improve eye
care worldwide.
Dr. Foster is a happily married, father of three teenagers. He spends most of his free time with his family and church
and loves gardening, fishing, biking, photography, water sports, and hunting.

Kelly Herschel, MD is board certified and fellowship trained ophthalmologist. He is a specialist in cornea, refractive surgery
and oculoplastics and brings his great diversity of skills directly to his patients. Dr. Herschel graduated magna cum laude from
Vanderbilt University and went on to receive his medical degree from the University of Oklahoma. He completed his
residency at the Alton Ochsner Medical Foundation in New Orleans, where in his senior year, he served as chief resident.
Following his residency at the Alton Ochsner Medical Foundation in New Orleans, Dr. Herschel completed an extra sub-
specialty fellowship in cornea, external disease, and refractive surgery at the Louisiana State University Eye Center. As chief
fellow at the Louisiana State University Eye Center, Dr. Herschel coordinated the stage III FDA trials on myopic PRK
(photorefractive keratotomy) and later, hyperopic PRK. Subsequently, Dr. Herschel joined a prestigious practice in Dallas
where he perfected his surgical skills and began performing LASIK procedures after the FDA approval of PRK in 1996. Prior
to joining The Magruder Eye Institute in 2000, Dr. Herschel, completed a third fellowship in oculoplastic surgery at Beraja
Eye Institute in Miami. He has helped many patients achieve their goal of better vision through his wealth of ocular
knowledge and his dedication to continual learning.
Dr. Herschel is the director and founder of Herschel LASIK at Magruder Eye. He opened his practice with the understanding
that every patient deserves the very best vision correction results. He has focused his efforts on sustaining a practice which
provides an advanced level in technology in vision correction combined with unparalleled patient care and experience. Dr.
Herschel understands the process in deciding to have vision correction is not one that happens overnight for most people. He
believes it is vital he follow his patients through the entire pre and post operative process. Dr. Herschel and his team perform
all testing prior to surgery and Dr. Herschel personally follows his patients through their post operative visits. All laser vision
correction procedures are performed on site at Herschel LASIK and other complex eye surgeries are most often performed in
the surgery center at Magruder Eye Institute.
Dr. Herschel practices general ophthalmology while specializing in comprehensive refractive surgery including LASIK,
oculoplastic problems, cataract and complex corneal surgery. He continues to incorporate new and exciting technologies into
his practice including Intra Ocular Lenses, high contrast implants and DSAEK. While you may be a candidate for LASIK,
there are now advanced procedures which may be available to you. Dr. Herschel will recommend the vision correction option
which makes most sense for you.

Dr. Kassar is a board-certified hematologist and medical oncologist. He completed his fellowship training at
Rush University Medical Center, where he served as a clinical assistant professor of medicine. His cancer
research work has been widely published in numerous specialty medical journals, and he has written on a
number of different types of cancer.
Dr. Kassar is proud to have served the community of northwest Indiana for the last decade. Dr. Kassar co-
directs the Cancer Center at Community Hospital in Dyer, and he serves as the principal investigator for more
than 15 clinical trials funded by the National Cancer Institute. He was the first oncologist in the area to offer
state-of-the-art immunotherapy in the treatment of various types of cancers, utilizing this approach before they
became FDA standards of care. His focus is to incorporate the latest advances in cancer treatment into his
clinical practice. Dr. Kassar places an emphasis on the importance of preserving the quality of life and providing
strong support for his patients and their families throughout their cancer treatment journey.

A resident of Grand Rapids, MI, Dr. Keil is a Board-Certified surgeon by the American Osteopathic Board of
Ophthalmology, and is a Fellow of the American Osteopathic College of Ophthalmology. He has been involved
in Lasik surgery since 1999 and was the Medical Director and Lasik Surgeon at LasikPlus in Grand Rapids,
which opened in 2006. Dr. Keil enjoyed it so much that in 2009 he bought the center, which is now Keil Lasik
Vision Center.
With his long and highly successful career as a Lasik surgeon and having performed almost 10,000 surgeries, he
is ranked as one of Michigan’s most experienced. As such, professionals such as firefighters, law-enforcement,
doctors and media personalities have sought out Dr. Keil to perform their Lasik procedure.
In 2015 Dr. Keil became the first surgeon in the state of Michigan to perform the revolutionary Kamra™
corneal inlay procedure, for the treatment of presbyopia (near vision loss). This procedure is designed to reduce
or eliminate a person’s need for reading glasses..
As Dr. Michael Keil has a strong dedication to teaching, outside of his practice, Keil Lasik Vision Center, he
keeps busy as the Ophthalmology Residency Program Director at Metro Health Hospital. At Metro, Dr. Keil is
part of a teaching and research group that provides full vision care services, and also teaches medical students,
interns, and residents.
Both Dr. Keil and his wife have had laser vision correction done on their eyes. He says the procedure has greatly
helped him do the things he loves, such as sailing, skiing, and swimming.
Dr. Keil and his wife have three boys, and various pets: a crayfish, fish, a guinea pig, and last but not least a
parakeet named Mustard. In his free time, Dr. Keil enjoys hitting the trails with his mountain bike, camping,
staying active with his boys that are in Boy Scouts and spending his summer days on the water.

John Kempen, MD, MPH, MHS, PhD is an internationally renowned academic ophthalmologist and researcher
specializing in inflammatory disorders of the eye (a specialty completely unrepresented in sub-Saharan Africa
including South Africa). His research has resulted in over 100 peer-reviewed scientific publications, and he
continues to lead multiple large-scale US and international research projects.
Dr Kempen formerly directed the Public Health Ophthalmology Program at Johns Hopkins, one of the two
leading international ophthalmology programs in the world. He serves as Associate Editor-in-Chief of
Ophthalmic Epidemiology and on three additional editorial boards, including as Executive Editor emeritus for
the ocular inflammation section of the American Journal of Ophthalmology.
Dr Kempen is a Professor of Ophthalmology and Epidemiology at the University of Pennsylvania where he
serves as Division Director for Ocular Inflammation and for Ophthalmic Epidemiology and International
Ophthalmology.

Meadow, Long Island, New York and went on with his fellowship training in Colon & Rectal Surgery at Lehigh

Tirso M. Lara, MD specializes in the diagnosis, treatment, and care of patients with retinal disorders, including
special interests in macular degeneration, diabetic retinopathy, uveitis, and severe ocular trauma. He routinely
uses state–of–the–art surgical and laser techniques for various conditions such as retinal detachments, macular
holes, and eye injuries.
Dr. Lara is Board certified by the American Board of Ophthalmology. He is fluent in both English and Spanish.
Born in Washington D.C. and raised in Mexico, Dr. Lara graduated from LaSalle University School of Medicine.
He then completed a surgical research fellowship at Harvard Medical School and the Brigham and Women’s
Hospital in Boston, Massachusetts. During this time, he was the recipient of the National Research Service
Grant Award from the National Institutes of Health (NIH). He also received the Society of Black Academic
Surgeons (SBAS) grant award to support research.
Following a surgical internship at the University of Illinois at Chicago, he continued his residency training at
Vanderbilt University Department of Ophthalmology in Nashville, Tennessee. He furthered his studies with a
two–year fellowship in diseases and surgery of the retina and vitreous at the University of Alabama at
Birmingham and Retina Specialists of Alabama. As a fellow he learned the latest and most advanced methods of
treatment for retinal diseases. Dr. Lara also served as a Clinical Instructor of Ophthalmology for the Department
of Ophthalmology and was actively involved in teaching residents.
He is a member of the American Academy of Ophthalmology, the American Society of Ocular Trauma, the
Florida Academy of Ophthalmology, and the Broward County Ophthalmological Society. Dr. Lara is board
certified by the American Board of Ophthalmology.

Scott Lawrence, MD, MDiv, ThM is an academic ophthalmologist specializing in glaucoma, the second leading cause
of blindness in the world. With extensive international experience, he currently serves as Assistant Professor of
Ophthalmology and Division Director for Glaucoma at the University of North Carolina at Chapel Hill, USA.
Dr Lawrence is a nationally recognized expert and lecturer on cataract surgery and the management of glaucoma, with
numerous peer-reviewed articles in his specialty.
An experienced educator, Dr. Lawrence is heavily engaged in clinical and surgical ophthalmology resident education.

David Litoff, MD is an ophthalmologist in Denver Colorado. He is currently the Chief Medical Officer at Icon Eyecare,
a large eyecare group currently in Colorado, Arizona, and Texas. David Litoff grew up in Connecticut and after
completing medical school at the University of Connecticut he went on to complete his ophthalmology residency at the
Albany Medical Center in Albany New York. After completing his residency he went to Iowa to complete a cornea

Michael H. Manning, MD, FACS is a board-certified ophthalmologist practicing in the Tampa, Florida area. He is a

Lisa Martén, M.D. is Board Certified by the American Board of Ophthalmology with advanced fellowship
training in Cornea and Refractive Surgery. Dr. Martén received her undergraduate degree from Tulane
University, as well as attending their School of Public Health Tropical Medicine for her Master’s Degree in
Nutrition. She also earned her Medical Degree from their medical school in 2001. During her studies, she won
the AMA Foundation Leadership Award for her work as the president of the Latin American Medical Student
Association. She also participated in research and received a grant from the American Heart Association.
After completing her internship, Dr. Martén came to San Antonio for her Ophthalmology residency at the UT
Health Sciences Center where she is now an assistant clinical professor. She moved to Tennessee where she
attended the Wang Vision Institute for her fellowship. Dr. Lisa Martén is currently the medical director and
owner of the South Texas Eye Institute, the first clinic in San Antonio to perform Lasik. South Texas Eye

Dr. Matthys completed an orthopaedic residency at the University of Southern California and Los Angeles
Medical Center in California. Following this training, Dr. Matthys was awarded 3 prestigious fellowships in joint
replacement and reconstructive surgery at Harvard Medical School and Brigham and Women's Hospital. He was

Frederico Mattioli, MD is board-certified by the American Board of Physician Specialties in Ophthalmology. He has
performed hundreds of cataract/implant procedures and 35,000 laser refractive treatments, as well as cosmetic proce-
