Welcome!

The PHYSICIAN CEO network is a valuable feature of the program. Please use this confidential directory

of the 2022 PHYSICIAN CEO® class to learn about the program participants and to foster communica-

tions throughout the year. As you will see, the class of 2022 is an impressive collection of extraordinary indi-

viduals drawing from 6 continents, 10 specialties, and a broad span of ages.

The PHYSICIAN CEO® program was developed by SurgiVision Consultants, Inc. in collaboration with the

Kellogg School of Management at Northwestern University. We are fortunate to have a partner in Kellogg,

which is consistently ranked among the top programs in the world for Executive Education. Most of all, we

are fortunate to work with physicians of such high caliber as those in the 2022 PHYSICIAN CEO program.

Globally acclaimed faculty, a focus on CEO leadership, and a physician-oriented curriculum combine to

make the PHYSICIAN CEO program unique. As we embark on the first module, we begin of a journey

that will lead to growth, new visions and lasting friendships.

Norms and Expectations

Participants are expected to conduct themselves within acceptable norms of behavior and conduct at all times throughout the duration of the program, both on and off the campus of the James L. Allen Center and the Kellogg School of Management at Northwestern University.

DRESS CODE AND CONDUCT

Dress code is business casual.

• The PHYSICIAN CEO program is designed for professional development and interaction. Disruptive, abusive, or disrespectful behavior is prohibited and may lead to expulsion from the program at the sole discretion of the program administrators.

• Please wear your lanyard and name tag at all times when in the Allen Center

KELLOGG EXECUTIVE EDUCATION COVID-19 SAFETY PLAN

• Kellogg requires participants in classroom programs to be fully vaccinated OR complete COVID-19 testing prior to arrival and during course of the program. • Additionally, “Masks” are required in all indoor spaces unless actively eating or drinking of while in your private suite.

PUNCTUALITY, PARTICIPATION AND ATTENDANCE

• All participants are expected to actively participate in both classroom and group activities.

• Due to the carefully planned nature of the program, all participants are expected to arrive at the scheduled classes and activities on time. Late arrival may prevent entry until the next session.

CELL PHONE AND ELECTRONIC DEVICE USE

• In-class use of cell phones and computers for non-classroom related activities is prohibited. Participants who must take emergency calls are asked to leave the classroom. Re-entry into the classroom may be denied and is left to the discretion of the faculty. • No audio or videotaping of any kind is permitted of any part of the PHYSICIAN CEO program. Program Chairman

Guy M Kezirian, MD, MBA, FACS is the president of SurgiVision® Consultants, Inc. of Scottsdale, Arizona. He is a board-

certified ophthalmologist and a Fellow of the American College of Surgeons. He earned his MBA from Northwestern

University’s Kellogg School of Management, and also holds an American Society of Ophthalmic and Plastic Surgery (ASOPRS)

specialty board certification. Dr. Kezirian obtained his BSc from McGill University and his MD from the Brown University

Program in Medicine.

SurgiVision® Consultants, Inc. was founded in 1995 and is located in Scottsdale, Arizona with a mission to contribute to the

advancement of refractive surgery and ophthalmology through innovation. Over the years, SurgiVision® has fulfilled that

purpose in many dimensions, including:

· Design and administration of many successful FDA projects

· Work with manufacturers to develop new product designs

· Provision of custom, cloud-based OEM software products to monitor the safe release of new products

· Development of the SurgiVision® DataLink family of software products

· Development and administration of some of the largest privately held clinical registries for ophthalmic data

· Development and program direction for professional meetings

Prior to founding SurgiVision® Consultants, Inc., Dr. Kezirian started the first practice in the United States devoted exclusively

to refractive surgery in 1991, and has worked extensively throughout the ophthalmic industry. He has authored more than 20

articles for the peer-review literature and serves on the editorial advisory board of several ophthalmic trade journals.

SurgiVision®’s extensive network of affiliate consultants includes regulatory experts, software engineers, IT experts,

academics, and attorneys.

In addition to his work with SurgiVision®, PHYSICIAN CEO™ and the Refractive Surgery Alliance, Dr. Kezirian serves as a

board member of the Himalayan Cataract Project, a 501(c)(3) organization with the mission to eliminate needless blindness on

a global scale.

Academic Director

For over 30+ years, Thomas M. Turmell has provided capital and advisory services to small, medium, and

growth businesses. His background includes diverse experience managing financial and operating aspects of

companies, including: investment/capital deployment analysis and execution; financial planning; strategic

planning; corporate structuring; investor negotiations/relations; lender relationship management; vendor

negotiations; and leading continuous improvement initiatives. Mr. Turmell is a Managing Director at TMT

Capital Partners, LLC and Director of Corporate Partnerships for Executive Education at Kellogg School of

Management at Northwestern University.

Prior to founding TMT Capital, Mr. Turmell held the position of Principal at Golub Capital, providing senior

debt, junior debt, and equity capital to middle market companies. Mr. Turmell joined the firm in 2004 to

establish and build the firm’s Chicago office, make new investments, and participate in portfolio management.

Before Golub Capital, Mr. Turmell was a Vice President with Pfingsten Partners, LLC., an operationally –

oriented, middle market private equity firm focused on making control investments in middle market companies

in the manufacturing, distribution, and publishing sectors. Before Pfingsten, Mr. Turmell was a Vice President at

LaSalle Bank, NA, providing credit and non – credit financial solutions to middle market manufacturing,

distribution and service businesses.

In addition to his activities at TMT Capital, Mr. Turmell is an Executive Committee Member of the Chicago

Chapter of the Association for Corporate Growth, and leads several committees that execute various

conferences and programs produced by the Chapter. He also serves as an Advisory Board member of Windsohy,

LLC, and DealRoom.

A Michigan native, Mr. Turmell holds a BBA degree in Business Administration from the University of Notre

Dame, and an MBA degree in Finance, Management and Strategy, and Organizational Behavior from the J. L.

Kellogg Graduate School of Management.

Nathan Abraham, MD is the Founder and CEO of Abraham Eye Center in Valencia, California. He is board-

certified cataract, cornea, and refractive surgeon. Nathan's roots are in Southern California, where he was raised

and has many relatives and friends. His education and training have taken him too various parts of the country

including Washington DC, Orlando, FL, and back to Los Angeles, CA, where he completed his fellowship

Juan F. Batlle Logroño, MD is the Vice-President of Centro Láser in Santo Domingo in the Dominican Republic,

where he specializes in Corneal Transplant surgeries(DALK, DSAEK, DMEK, PK), Refractive Surgery (LASIK,

PRK, SMILE), Cataract Surgery, Neuro-Ophthalmology, Correction of Refractive Defects with intraocular lens

(ICL), Dry Eye and Eye Surface disease, and the use of Amniotic Membrane in corneal problems.

Dr. Batlle is the Vice-President of Banco de Córneas in the Dominican Republic, a non-profit organization that

seeks to promote the donation of eye tissues for people with corneal conditions. Thanks to this work, the

organization has made innumerable transplants, promoting the process of awareness about the importance of being

a donor.

“I always had my vocation clear. I learned it from my father, seeing his commitment to patients. I am the third

generation of a family of ophthalmologists, and I know the responsibility that implies for me.”

As a doctor specialized in different areas of Ophthalmology, Dr. Batlle Logroño collaborates with the other doctors

of Centro Láser in the visual health operations of the organization. As Vice President of the Corneas Bank of the

Dominican Republic, he is one of the main promoters of the donation of organs for the sight, making people aware

of the importance of the cornea for a correct visual health and the human and social commitment that implies the

donation process. As Vice President of Centro Láser, he focuses on promoting the investigation of new areas and

modern techniques in Ophthalmology, in order to offer a better service for the visual health of Dominicans.

Dr. Batlle studied at the Vanderbilt University, where he obtained his Bachelor’s Degree in Neuroscience. He went

on to obtain a Doctorate in Medicine from the Tulane University School of Medicine, where he also completed his

Residency in Ophthalmology. Subsequently, he studied to specialize in the area of cornea and refractive surgery. As

an ophthalmologist recognized for his work, Dr. Batlle is a member of several important organizations focused on

the development of Ophthalmology at both national and international levels. Currently, he is a member of the

Dominican Ophthalmology Society (SDO), the Pan American Ophthalmology Association (APO), the American

Society of Cataract and Refractive Surgery (ASCRS), the American Academy of Ophthalmology (AAO), and the

Refractive Surgery Alliance Society (RSA). He has made several publications in specialized journals in

Ophthalmology, such as the article “Case Reports in Ophthalmological Medicine” in 2016.

Adil Bhatti, MD is a Clinical Assistant Professor in the Faculty of Medicine at the University of Ottawa. He

obtained his Medical Degree from the University of Toronto in 2009. Following medical school, Dr. Bhatti

completed his residency training in Ophthalmology at the University of Ottawa Eye Institute where he served as

Chief Resident.

Dr. Bhatti joined the Department of Ophthalmology at The University of Ottawa Eye Institute in 2014. He has

taken on the role as Medical Educator and is passionate about teaching both medical students and residents.

That same year in 2014, Dr. Bhatti co-founded iCare Surgical Centre that helps serve the community of Ottawa.

After three years of practice Dr. Bhatti went on to complete a Medical Retina fellowship at Queen’s University

in Kingston in 2018. Dr. Bhatti specializes in diseases of the Retina and Cataract Surgery.

Outside of work Dr. Bhatti loves to spend time with his family, play sports such as hockey, skiing, exercising and

traveling.

Brett P. Bielory, M.D., is the new Medical Director of our Ophthalmology services at Riverside Medical Group,

and specializes in corneal transplantation, refractive surgery and ocular surface disease. Dr. Bielory received his

undergraduate degree from the University of Michigan and his doctorate in Medicine from the Technion-Israel

Institute of Technology. He completed an Ocular Pathology research fellowship at the Bascom Palmer Eye

Institute in Miami, Florida. He completed his ophthalmology residency at New York Medical College.

Dr. Bielory finished his training by completing a fellowship in cornea and refractive surgery fellowship at Tufts

University/New England Eye Center. He implements a customized, patient-centered approach incorporating

surgical and non-surgical options to help patients choose their best treatment plan for corneal, refractive, and

functional concerns of the eyes. Dr Bielory is an innovator and enthusiastic teacher who loves bridging the gap

between basic science, clinical practice and patient compassion. He enjoys teaching residents and fellows in the

clinic and operating room at the New York Eye and Ear Infirmary.

Dr. Bielory has published over 20 articles and book chapters in the field of ocular surface disease with a keen

interest in ocular allergy. He is a reviewer for numerous nationally peer-reviewed journals such as JAMA

Ophthalmology and British Journal of Ophthalmology. As Director of Ophthalmology at Riverside Medical

Group, Dr. Bielory combines his clinical care passion, scientific drive and innovative creativity to provide first

class comprehensive care with a focus on Ocular Surface Disease management.

Ross A. Clevens, MD, MPH, FACS, is a facial plastic and reconstructive surgeon practicing in Brevard County

Central Florida. Dr. Clevens is the founder of Clevens Face & Body Specialists, a cosmetic surgery group

practice comprised of four board certified plastic surgery specialty physicians as well as mid-level providers

offering comprehensive care in both plastic surgery and facial plastic surgery. The practice focuses on cosmetic

surgery of the face, breast and body, post-MOHS facial reconstruction and is home to the most advanced

aesthetic laser systems in Central Florida. The practice has thirty employees with locations in Melbourne and

Merritt Island, Florida located about one-hour east of Orlando along the Atlantic Coast.

Dr. Clevens is respected as a key opinion leader and has advised and taught on behalf of many companies in the

cosmetic surgery and aesthetic laser spaces. He is frequently invited to share his experience by teaching and

lecturing about both facial plastic surgery and practice management in the US and abroad.

His lifelong passion of philanthropy led Dr. Clevens to create and found Face of Change, a 501(c)3 nonprofit

foundation that provides safe water, food and medical care to those in need both at home and in East Africa.

Face of Change has provided nearly one million meals to the hungry in Florida and abroad. As part of his work

with Face of Change, Dr. Clevens has led humanitarian and medical missions to Tanzania, Kenya, Israel and

Jordan.

Dr. Clevens is a graduate of Yale University and Harvard Medical School and holds an MPH in Health Policy

and Management, also from Harvard. He completed his residency in otolaryngology-head and neck surgery and

fellowship in facial plastic and reconstructive surgery at The University of Michigan. Dr. Clevens is double board

certified in both otolaryngology and facial plastic and reconstructive surgery. Dr. Clevens is on the faculty of the

University of Central Florida College of Medicine and is director of the fellowship program in facial plastic and

reconstructive surgery at Clevens Face & Body Specialists.

Dr. Clevens is the author of Your Plastic Surgery Companion: A consumer’s guide to facial plastic surgery. Dr.

Clevens is the married father of two amazing grown boys and is an avid photographer who loves travel and

enjoys CrossFit.

Paul E. Cutarelli MD is a refractive and cataract specialist who has been in practice in Denver, CO for 20 years.

He founded Cutarelli Vision in 2008 as a refractive, cataract and corneal practice and it now shares 3 satellite

offices. Dr. Cutarelli practiced 3 years as the Director of Refractive Surgery at University Hospitals of Cleveland

and Assistant Professor of Ophthalmology at Case Western Reserve University School of Medicine. He was the

founder and Director of the first Corneal and Refractive Fellowship and member of the Cornea Service for the

Department of Ophthalmology at University Hospitals and Case Western Reserve. Dr. Cutarelli completed his

residency training at The Johns Hopkins Hospital, and completed his Refractive & Anterior Surgery fellowship

with Daniel Durrie, MD and a Corneal & External Disease Fellowship at the Moorfields Eye Hospital in

Drew Dickson, MD is currently completing his refractive surgery fellowship at Kugler Vision in Omaha, NE .

Dr. Dickson developed an interest in refractive surgery during his ophthalmology residency, where he was first

exposed to the joy of improving the lives of his patients with refractive surgery.

Dr. Dickson enjoys partnering with his patients to develop customized plans to improve their vision and lives.

He takes a patient-centered approach to focus on the individual needs of each of his patients while utilizing state

-of-the- art technology to achieve his patients’ goals. Dr. Dickson is motivated to help each of his patients

achieve their best vision possible, and he loves to see his patients’ lives dramatically improved through refractive

surgery. He also has an interest in international ophthalmology and desires to help improve vision of people all

over the world through eye surgery.

Dr. Dickson is originally from Clear Lake, IA. He attended Liberty University in Lynchburg, VA, where he

graduated summa cum laude with a degree in Biology. He then received his medical degree from the University

of Nebraska Medical Center College of Medicine in Omaha, NE, where he received membership to the Alpha

Omega Alpha Honor Medical Society. After graduating from medical school, Dr. Dickson completed his

internship at the University of South Dakota in Sioux Falls, SD. He then went on to complete his

ophthalmology residency training at the Storm Eye Institute at the Medical University of South Carolina in

Charleston, SC.

Outside of ophthalmology, Dr. Dickson enjoys spending time with his wife and son. He enjoys outdoor

activities, cooking, and sports, especially Nebraska Cornhusker football.

Anna Edmiston, MD, is a refractive, cornea, and cataract surgeon in Oklahoma City, OK. She was born in

Alabama and is an Auburn Tiger. WAR EAGLE. She trained at UC Denver for residency, Jacksoneye in Illinois

for fellowship, and then joined a high volume refractive surgery practice in Oklahoma City, OK in 2019. At

Anthony Grillo, MD is a board-certified ophthalmologist specializing in cornea, external disease, and refractive

surgery. He is an Oregon native who grew up in Southern Oregon and was always interested in medicine and

healing. He dabbled early starting with emergency services while in still in high school and became truly invested

while traveling shortly thereafter.

As a medical student at UCLA, Dr. Grillo became fascinated with microsurgery and the amazing results that eye

surgery could provide. During his residency at the Casey Eye Institute, he developed a passion for the intricacies

of surgery in the anterior segment of the eye. He took that interest and passion to the Mason Eye institute where

he honed his skills in cataract surgery, corneal transplants, and ocular reconstruction during his cornea

fellowship. He stayed on there on as faculty, teaching residents and fellows for the next three years before

recently returning home to Oregon to practice at Pacific ClearVision Institute in Eugene.

Dr. Grillo has consistently been viewed as the “Mr. Fix - it” regarding eye care. Whether it is reconstruction of the

eye after trauma, complicated cataract surgery, or challenging cases regarding a variety of corneal transplantation

techniques, he is always up for the challenges of the most difficult of cases. In order to meet his patient’s needs,

he is proud to provide laser refractive surgery, cataract surgery, and a variety of corneal transplants such as

DMEK, DSAEK, DALK, and PKP.

In addition to patient care, Dr. Grillo is passionate about teaching. He has instructed cornea fellows, as well as

numerous residents, acting as assistant program director for the residency program at the Mason Eye Institute.

Robert L. Harris, MD is a specialist in Female Pelvic Medicine and Reconstructive Surgery in Jackson,

Michael Herion is a Lt. Col. in the United States Air Force Reserve, representative of the desire to serve that

motivated his choice to go into the field of eye medicine. During his service time, he received the Air Force

Commendation Medal for his service in The Global War on Terror.

Dr. Herion attended Arizona State University for undergraduate studies before earning his doctorate from Saint

George’s University School of Medicine. His residency in ophthalmology took place at Saint Louis University

before he completed a retina fellowship with The Retina Group Ltd.

Dr. Herion enjoys spending time outside, including skiing and scuba diving. On fall Saturdays, Dr. Herion can

be found in the stands of Arizona State Sun Devils stadium cheering on his team.

Lisa Hunsicker is a board-certified plastic surgeon who has been practicing her "art" since 1995. Dr. Hunsicker decided

to pursue the specialty of plastic surgery in Denver specifically because she is committed to helping those in her care be

their most confident and positive selves.

Shalesh Kaushal, MD, PhD obtained his BS degree at Yale University in Molecular Biophysics and Biochemistry. Subsequently, he completed his MD

at Johns Hopkins and PhD at MIT with the Nobel Laureate Dr. Har Gobind Khorana. Following his PhD, he completed his residency at the Doheny

Eye Institute/USC Department of Ophthalmology and his surgical retina fellowship at the Washington University St. Louis/Barnes Retina Institute.

He spent an additional year at Moorfields Eye Hospital and the Institute of Ophthalmology in London.

While at the University of Florida, he was the Richardson II Chair and head of the retina division. Dr. Kaushal established the clinical and surgical

retina division, recruited physicians, trained residents and surgical fellows mentored undergraduates, medical students, graduate students, and post-

docs in his research lab. Further, he organized an annual retina symposium that attracted many key leaders both in the US and internationally. During

that time, he established and became the Director of the Charlie Mack Overstreet Laboratory for Retinal Therapies. In 2009, he was recruited to the

University of Massachusetts as the chairman to build a new eye center and the first new academic Department of Ophthalmology in the United States

in over 25 years. Besides these academic pursuits, he is the founder of three biotech companies and has been a consultant for most of the leading

ophthalmic pharmaceutical companies. In 2013, Dr. Kaushal transitioned back to Florida to pursue private practice.

The thrust of Dr. Kaushal’s basic research is to understand the cellular, molecular and biochemical events that cause retinal and macular degeneration.

Collectively, the major research projects in his lab couple biochemical, small molecule and genetic approaches to understanding these important

retinal diseases. Some of his work has led to novel physician initiated clinical trials. Dr. Kaushal was the first in the world to use four of the new drugs

in the management of retina patients.

More recently, his research has lResearch in Vision and Ophthalmology (ARVO), American Academy of Ophthalmology (AAO), the Macula Society,

American Ophthalmological ed to the identification of certain nutraceuticals to mitigate common retinal diseases like macular degeneration and

diabetic retinopathy. This has led him to further understand the direct role of nutritional biochemistry in controlling the structure and function of the

retina. He is also actively developing stem cell treatments for retinal diseases.

Three years ago, he co-founded a nov el nutraceutical company, Rheostasis, based on his discoveries and desire to treat diseases at their root

biochemical cause. This company has developed novel formulations for eye health, sugar metabolism, vascular health, and preserving and optimizing

cellular health.

Dr. Kaushal has served on the scientific board or as a consultant for many of the major pharmaceutical and ophthalmic companies. He is a consultant

for multiple other biotechnology and diagnostic device companies.

He has a currently funded research project studying the incidence of diabetic retinopathy in the general population using a novel telemedicine

instrument. Separately, he has an ongoing research collaboration with the University of Florida, Department of Pediatric Endocrinology.

Dr. Kaushal is a member of the Association of Society (AOS), Retina Society, the American Society of Retina Specialists (ASRS), the American Board

and Academy of Regenerative Medicine, the American Diabetes Association. He is also Fellow of the American College of Nutrition (ACN). In 2015,

Dr. Kaushal received the prestigious Seelig research award from the ACN. We is recognized in Marquis’s Who’s Who in Medicine and Science and

nominated by Castel Connolly as a Top Doctor. He is board certified by the American Board of Ophthalmology and the American Board of

Gary Mendese, MD, FAAD, FACMS is a Boston-hospital trained dermatologist and Mohs surgeon. Dr.

Mendese is double board certified in Dermatology as well as Micrographic Dermatologic Surgery, and specializes

primarily in Mohs micrographic surgery. He sees patients in both Massachusetts and New Hampshire. Dr.

Mendese completed his dermatology training at the combined Boston University/Tufts University Dermatology

Residency Program, where he served as Chief Resident. He completed a rigorous American College of Mohs

Surgery (ACMS) fellowship in Mohs Surgery in 2012 under the leadership of Donald J. Grande, MD, who was a

recognized leader in the field. He completed a post-sophomore fellowship in pathology during medical school

from 2004-2005 and was elected to the Alpha Omega Alpha honor society in his final year of medical school.

Currently, along with his daily practice duties, he is an Assistant Professor of Dermatology at the Tufts

University School of Medicine. He also serves as Vice President on the Board of Directors for the Massachusetts

Academy of Dermatology (MAD) and sits on the Advisory Board for Impact Melanoma (formerly Melanoma

Brett H. Mueller II, D.O., Ph.D. is a Board Certified, fellowship-trained refractive and cataract surgeon. He is a

6th generation Texan and a 4th generation native of San Antonio. After receiving his undergraduate degree from

Texas A&M University, he then received his medical degree and Ph.D. from the University of North Texas

Health Science Center as he worked to develop new treatment modalities for patients in the field of

ophthalmology.

In conducting neuroscience and pharmacology research of the eye in hopes of discovering a neuroprotective

agent to treat glaucoma, he was drawn to the complexity and beauty of the visual sciences. Dr. Mueller’s

scientific work and academic accomplishments enabled him to author numerous publications and book chapters,

perform over 30 presentations at national and international meetings, and is a recipient of several grants

(including an NIH grant). He was then accepted to attend an ophthalmology residency at the University of

Louisville, where he developed a reputation for being a leader amongst his colleagues, serving as chief resident,

and a skilled surgeon. After finishing residency training, Dr. Mueller then completed a one year refractive,

cataract, and anterior segment fellowship with nationally and internationally recognized vision correction

pioneer, Dr. Greg Parkhurst. With advanced fellowship training in refractive surgery, he hones a comprehensive

range of skill sets needed to diagnose, address, and treat the full spectrum of refractive surgery. He now

specializes in state of the art laser guided cataract surgery and advanced lens implants that reduce or eliminate

the need for glasses post-operatively. He also specializes in the full spectrum of refractive surgery which

includes: modern LASIK, SMILE, ICLs, Refractive Lens Exchange, PRK, and Cross-Linking. Now as an
